Work Description
Describe the hot work to be performed, equipment used, and materials involved.
Pre-Work Safety Checklist
The permit issuer must verify ALL items before authorizing hot work. Mark Yes / N/A / No for each item.
🏗 Area Preparation
All combustible/flammable materials within 35 ft (10.7 m) of work have been removed or protected with fire-resistant covers (NFPA 51B §4.3.2).
Floor openings, wall openings, cracks, and ducts within 35 ft have been covered or sealed to prevent spark/slag travel.
Combustible walls, partitions, ceilings, or roofs adjacent to work area have been wetted down or protected with fire-resistant blankets.
Adjacent areas on other side of walls/floors have been inspected and cleared of combustibles (sparks can travel through walls).
🌬 Atmosphere & Flammable Gases
Atmosphere tested for flammable/explosive gases and vapors — confirmed below 10% LEL (Lower Explosive Limit) before starting.
Flammable gas containers, tanks, and piping in the area have been purged, blanked, or removed. No flammable liquids or gases within 35 ft.
Ventilation is adequate to prevent accumulation of fumes and gases. Local exhaust ventilation in place if welding on coated or treated metals.
🧯 Fire Protection Equipment
Fully charged fire extinguisher (appropriate class) is immediately available at the hot work site and fire watch knows its location.
Sprinkler system is operational (not impaired or shut off). If impaired, hot work cannot proceed without additional protection per NFPA 25.
Fire alarm system is functional. Nearest manual pull station location confirmed by fire watch and hot work operator.
👁 Fire Watch Requirements
Trained fire watch assigned and present during all hot work operations. Fire watch has no other duties during hot work.
Fire watch will continue for minimum 60 minutes AFTER hot work stops (NFPA 51B §8.3). Post-work fire watch person identified and assigned.

Fire watch must sign in/out. Post-work watch must cover minimum 60 minutes after hot work ceases.
