1 · Fall Protection Fatal Four
1926.502Fall protection provided at all unprotected edges, floor holes, and openings where workers could fall 6 ft or more. Guardrails, safety nets, or personal fall arrest systems in use.

Guardrail systems in place: top rail 42 in ±3 in high, mid-rail, toe boards on all open sides of platforms and elevated walkways.

Personal fall arrest equipment (harness, lanyard, anchor) inspected before use, properly fitted, and attached to adequate anchorage (5,000 lb capacity per employee).

Roofing work: fall protection for low-slope roofs (slope ≤4:12) includes warning line system ≥6 ft from edge AND safety monitoring or fall arrest. Steep-slope: guardrails or PFAS.

1926.451–454Scaffolding designed, erected, and inspected by a competent person. Scaffolds rated for at least 4× the intended load. Capacity posted or communicated to workers.

Barricades, barriers, and warning signs in place around overhead work zones. Overhead protection (catch platforms, nets, canopies) provided where workers must pass below overhead operations.
