Lockout / Tagout Procedure

DANGER — Hazardous Energy: This procedure must be followed exactly. Failure to properly lockout/tagout equipment can result in serious injury or death. Only authorized employees may perform this procedure. Never bypass or remove another worker's lock.

Notify all affected employees that maintenance/servicing is beginning and the equipment will be locked out.

Equipment has been shut down using normal stopping procedure. All moving parts have come to a complete stop.

All energy isolating devices have been physically operated to the SAFE / OFF / CLOSED position (disconnect opened, valve closed).

Lockout / tagout device(s) applied to ALL energy isolating points. Each authorized employee has applied their own personal lock.

Stored / residual energy has been released, restrained, or dissipated — including capacitors discharged, pressure bled, elevated parts blocked, springs released, fluids drained.

Tried to operate the normal operating control (press START) to verify equipment will NOT energize. Control returned to OFF/NEUTRAL after test.

Zero-energy state confirmed using appropriate test equipment (e.g., voltage tester, pressure gauge). Equipment is SAFE to work on.

Ensure all tools, materials, and non-essential items have been removed from the work area and equipment.

Verify all employees are safely positioned away from the equipment — no personnel are in the danger zone.

Guards and safety devices reinstalled and verified in place before re-energizing.

Each authorized employee removes their own personal lock and lockout device from the energy isolating point(s).

Notify affected employees that the lockout/tagout has been removed and equipment is being returned to service.

Re-energize equipment using normal procedures. Observe for proper operation before returning to full production.

Authorization & Sign-Off

This completed procedure must be retained on file for the duration of servicing and a minimum of 5 years per company records policy. Per OSHA 1910.147(c)(4)(i), machine-specific procedures are required when employees perform LOTO on equipment with multiple or complex energy sources.
