Part 1 — Energy Source Inventory
Identify ALL hazardous energy sources. Each source must have its own isolation point and lockout device.

Authorization & Sign-Off
This completed procedure must be retained on file for the duration of servicing and a minimum of 5 years per company records policy. Per OSHA 1910.147(c)(4)(i), machine-specific procedures are required when employees perform LOTO on equipment with multiple or complex energy sources.
